MemberTo enroll in Hamaspik Choice you must be:
- At least 18 years of age
- Reside in one of the covered counties; Rockland, Orange Sullivan, Dutchess, Putnam and Ulster.
- Be Medicaid eligible
- Able to remain in your home environment
- In need of community based long term care service
Covered Services:
- Care Management
- Personal Care Services
- Home Care; nursing, personal care aide, home health aide, physical therapy, occupational therapy, speech pathology, nutrition and medical social services
- Adult Day Health Care (attendance in a group setting for meals, activities, companionship, therapies, monitoring medical status)
- Durable Medical Equipment; Medical/Surgical Supplies, Prosthetics and Orthotics, Compression and support stockings, Enteral formula and nutritional therapy
- Personal Emergency Response Systems (enables you to call for assistance in an emergency without having to reach a telephone)
- Non-emergency transportation to and from your scheduled health-related appointments
- Podiatry (foot care)
- Therapy; Physical Therapy, Occupational Therapy and Speech Therapy
- Respiratory Therapy, including preventive, maintenance and rehabilitative airway-related techniques and procedures
- Nutrition Therapy
- Private Duty Nursing
- Home Delivered or Congregate Meals
- Social Day Care (attendance in a group setting for meals, activities and companionship)
- Social and Environmental Supports, including home improvements and modifications, appliances and assistive devices
- Nursing Home Care
- Dental Care
- Optometry Exams and Eyeglasses
- Audiology
- Consumer directed personal assistance services

- Inpatient and Outpatient Hospital Services
- Physician Services
- Laboratory Services
- Prescription, Non-prescription and compounded Prescription drugs

- Office for People With Developmental Disabilities (OPWDD)
